RISE Dementia CARE
Dementia changes things—but it doesn’t erase the need for connection and well-being.
Our approach is rooted in relationships. When we understand what’s happening, we can respond with confidence, not fear. When we learn practical, compassionate strategies, we can replace frustration with connection. And when we shift our focus from loss to ability, we create an environment where dignity, purpose, and autonomy thrive.
Whether you're a family member navigating a new diagnosis, a care professional seeking deeper insight, or a community looking to foster a truly dementia-friendly culture, you are not alone. Building awareness and hands-on skills, we empower you to transform the way you support and engage with those living with dementia.
Let’s RISE together—toward deeper understanding, greater confidence, and more moments of joy. By bridging the gap between knowledge and action, we can create meaningful, compassionate environments where those living with dementia can truly thrive.

My Story

The Heart Behind RISE Dementia CARE
RISE Dementia CARE is more than just my work—it’s personal. As a care partner to loved ones on the dementia journey, I’ve experienced firsthand the challenges, joys, and deep connections that come with supporting someone living with brain change. This work is full circle for me, and it fuels my passion for empowering others to navigate this journey with confidence and compassion.
My mission is simple yet profound: to empower care partners, families, and organizations to navigate the dementia journey. With over 15 years of experience in senior living and dementia care, I’ve dedicated my career to fostering dignity, connection, and well-being for individuals living with dementia and those who support them.
As a Certified Dementia Practitioner® (CDP), Certified Montessori Dementia Care Professional® (CMDCP), Certified Dementia Support Group Facilitator® (CDSGF), and Certified Patient Experience Professional (CPXP), I bring a deep understanding of the human experience of dementia and the best practices that support meaningful care. Through my work as an independent certified coach, trainer, and consultant with Teepa Snow’s Positive Approach to Care®, I provide strengths-based, relationship-centered education and support that is practical, compassionate, and effective.
Whether through hands-on training, consulting, or educational workshops, my goal is to equip care partners with the tools and confidence they need to create meaningful connections and deliver compassionate care. I believe that by focusing on strengths and fostering supportive environments, we can elevate the experience of living with dementia—not just for the individual, but for everyone involved.
Outcomes That Make a Difference
RISE Dementia CARE is committed to meaningful, measurable outcomes that enhance dementia support and care. Through experiential, skill-based training, care partners gain confidence in handling challenges, leading to reduced stress and greater resilience. Person-centered communication skills improve interactions, fostering stronger relationships and deeper connections. Within organizations, hands-on learning and coaching create cohesive teams that collaborate more effectively. Most importantly, individuals living with dementia and their care partners experience a better quality of life—where dignity, autonomy, and moments of joy are prioritized.
Dementia care is a journey, and no one should have to walk it alone. Let’s work together to create spaces where strengths are celebrated, relationships are nurtured, and well-being is prioritized.

"I actually thanked administration for this in-service. I learned so much. Helped me understand our residents and provided me the ability to improve myself and my job performance to what our residents need, not only with dementia, but all residents."

"Maureen Braen, of RISE Dementia Care, presented a great “Brain Healthy Living” event for our seniors. Maureen’s passion and knowledge shine through and make a sometimes difficult subject easy and engaging. The interactive seminar was suitable for all ages, and we learned new information about this valuable topic. Maureen brings a fresh and refreshing approach to the subject, with new ways to train our brain in a fun session! Highly recommended for any senior center, senior living facility, or community education."

"I appreciated having someone come alongside me in this journey and not feeling alone. However, more than that, although Maureen Braen was caring and knowledgeable, it meant a lot to me to know that she is actively walking this path herself at this time."

"Maureen has many practical ideas to solve problems. She had many "quotable quotes" too which I often think about and find myself having a better perspective when caring for my mom."
